This service supplements our in practice chronic disease management programmes for diabetes and is supported by Sanofi Aventis.

The Diabetes Nurse Specialist works solely with diabetic patients and their carers, helping them control, understand and manage their diabetes, and helping physicians care for diabetic patients. This service is aimed particularly at the chronic diabetic patient who would normally be treated in hospital. This is a weekly clinic and patients can be referred by all GPs in the area.
